What: Our case conference meetings allow providers to come together and collaborate on how best to serve complex clients as well as discuss HIV clients and consult about doing a SOAR claim.

If you have a client you would like to case conference, email Jan Ferdous by March 16 at jferdous@hchmd.org with:

• name of client
• Health Care for the Homeless number
• list of treatment providers
• reason for conference
• scheduling conflicts you have
• whether or not your case conference is a SOAR related case so that Jill, Lauren or Bilqis can be added to the provider list
