Navigating referrals just got easier

Navigating the complex referral process can be a full-time job in and of itself.

But our referrals committee—a group of Health Care for the Homeless staff members working together to improve the referral process—has got a solution! For one year, we'll have a referrals navigator at 421 Fallsway. At our downtown clinic, the referrals navigator will be dedicated to helping schedule referrals for clients who aren’t able to schedule appointments on their own.

The navigator will focus on clients who are cognitively impaired or who otherwise might have difficulty scheduling their own appointments. Our current referrals navigator, Lutheran Volunteer Coordinator Zoe Barlow, also speaks Spanish and can assist with Spanish speakers; she will use the language line for speakers of other languages.

If you have a client who needs assistance making an appointment for their referral at 421 Fallsway, please send a flag or instructions to the Unit Clerk CAAs in the EHR's check-out flag, and they will add that client to the referrals navigator’s schedule. If the referral can be processed that same day, the navigator will also be able to meet with the client in a same-day slot.

Zoe will be the referrals navigator until August 2018. Then, the referrals committee will determine whether a permanent position is needed and at which clinics.
